Access to Fitness Centre Facilities*
I acknowledge that Fitness Centre Membership will provide access to all facilities including exercise equipment and studio, 7 days a week from 6 am to 11 pm (exceptions will be notified by the office)
Cancellation Fee (Applicable for 12-month contract)
I acknowledge that in case of 12-month contract, should I choose to cancel the membership before the end of the contract, cancellation fees of 3 months shall apply (membership fee + tax)
FOB / Access Card*
I acknowledge that $10 deposit for FOB / Access Card is required. Fitness Centre Members are responsible for their own FOB/ Access Card. Shared gym access with anyone else (member or non-member) will result in immediate forfeiture of the Fitness Centre membership.
Safety Precautions*
While reasonable safety precautions have been taken in relation to the Fitness Centre, I assume the risk and responsibility for any loss or damage to the property or death or personal injury arising from my use of the Fitness Centre. ISIJ or its staff will not be responsible or liable for any loss or damage to the property or death or personal injury arising from the applicant’s use of the Fitness Centre. Please that you must consult a physician prior to using the Fitness Centre should you require.
